Participation Requirements

All student-athletes who participate in intercollegiate athletics at Martin Luther College must first have a completed physical exam, submit sickle cell test results, have proof of insurance, and submit all athletic forms before any participation (including conditioning or practice), and take time to review the information regarding the required forms. 

New athletes: Before your physical exam, you will need to know if you have already been tested for sickle cell trait (often as part of newborn screening). You can first check with the hospital where you were born. Otherwise, you will need this test done during your physical exam. Proof of these test results is required by the NCAA. Please print the MLC Physical exam form to take to your appointment.

The NCAA mandates a pre-participation physical exam (PPE) for all student-athletes before their initial season of eligibility. This exam must occur within 6 months of the first practice session, and must be administered by a qualified healthcare professional (physician, physician assistant, or nurse practitioner).

If you have a prescription for ADD/ADHD medication or an inhaler for asthma, you will need additional documentation from your physician. These drugs are considered banned substances, and supporting documentation is required. Please have your physician complete this
Doctor's Note, and save a copy to upload to your form.

These forms are all completed online. You may begin completing the required forms on or after
June 1st, and they are due by July 15th. See the links below. You will need a copy of your completed physical exam form and a copy of your insurance card (front & back) to upload as part of the online forms.

First Year Student Athletes and Transfer Student-Athletes: (Forms due by July 15th)

1. Physical Exam Form (You will need to print the form here and take it to your appointment. Please keep a copy for your records to upload when completing the online forms. This physical is good for 2 years).
